2021 Ethics in Action Award Nomination

Thank you for taking the time to nominate someone who has helped create healthy neighborhoods, workplaces, and/or schools in Brown County, Wisconsin – someone who makes our community a better place to live, work, and do business.

Nominations open until 11:59 pm June 13, 2020

Print the nomination form, and mail the completed form to Foundations (address on form) by the deadline, or
Submit a video or audio nomination. Video or audio nominations should contain all of the information required in the electronic form below and should be emailed to [email protected] by the deadline.

The ideal nomination focuses on a story, (i.e., a specific situation, concern or challenge in the community and how the nominee transformed this situation). We aren't looking for information about how amazing the nominee is as a human (we know they are!). The goal is to inspire action with their individual story.
